UFC 21: Return of the Champions was a mixed martial arts event held by the Ultimate Fighting Championship on July 16, 1999 at the Five Seasons Events Center in Cedar Rapids, Iowa. The event was seen live on pay per view in the United States, and later released on home video. Headlining the event was a Lightweight Championship bout between Pat Miletich and Andre Pederneiras.
UFC 21 marked significant rule changes; Preliminary bouts now consisted of two five minute-rounds, main card bouts were three five-minute rounds, and championship bouts were five five-minute rounds (identical to the Unified Rules of Combat used today in the USA). UFC 21 was also the first UFC event to adopt the 10-point must system, commonly used in boxing, to judge fights.

- Kosaka wins by TKO (doctor stoppage due to concussion) at 5:00 of the second round.
- Middleweight Bout: Paul Jones vs. Flavio Luiz Moura
- Jones wins by submission (rear naked choke) at 4:21 of the first round.
- Middleweight Bout: Jeremy Horn vs. Daiju Takase
- Horn wins by TKO (referee stoppage due to strikes) at 4:42 of the first round.
- Heavyweight Bout: Maurice Smith vs. Marco Ruas
- Smith wins by TKO (corner stoppage due to knee injury) at 5:00 of the first round.
- Lightweight Championship Bout: Pat Miletich vs. Andre Pederneiras
- Miletich wins by TKO (doctor stoppage due to cut) at 2:20 of the second round.
