Route 111 is both a north-south and east-west state highway in Massachusetts, though it is signed exclusively as a north-south route on newer signs. A few older east-west directional signs still exist east of Harvard, where the road changes direction. Route 111 is a continuation of New Hampshire Route 111.
From the state line between Pepperell, MA and Hollis, NH, Route 111 runs south through the following towns:
- Pepperell (sharing a small section of roadway with Route 119)
- Groton (sharing roadway with Route 119 and Route 225)
- Ayer (sharing roadway with Route 110 and Route 2A)
- Harvard (sharing roadway with Route 110)
Route 111 then proceeds east through the following towns:
- Harvard
- Boxborough
- Acton (sharing roadway with Route 2 starting at exit 43)
- Concord (sharing roadway with Route 2 and terminating at the State Prison Rotary at Route 2A and Route 119. Route 2 continues from this point into Boston.
Route 111 crosses the following Interstate highway:
- Interstate 495 in Boxborough (exit 28).
