Maryland Route 346 is a state highway in the U.S. state of Maryland. The route preserves the old alignment of U.S. Route 50 between Salisbury in Wicomico County and Berlin in Worcester County.

Maryland Route 346 begins at U.S. Route 50 Business just east of downtown Salisbury. It heads east through the eastern part of Salisbury on Church Street. It becomes known as Old Ocean City Road when it leaves Salisbury. Maryland Route 346 parallels US 50 to the north through eastern Wicomico County, passing through the towns of Pittsville and Willards. It continues to parallel US 50 to the north into Worcester County, but it eventually crosses US 50 and dives south and skirts the northern edge of the town of Berlin. Maryland Route 346 then returns to US 50 just east of Berlin.
Maryland Route 346 crosses two limited access highways: the U.S. Route 13/U.S. Route 50 Salisbury Bypass and Maryland Route 90, the Ocean City Expressway. However, Maryland Route 346 does not feature interchanges with either of these highways.
