Mafia primarily refers to the Sicilian criminal organization also known as Cosa Nostra or to its offshoot in the United States of America, sometimes simply referred to as the "American Mafia".

The word "mafia" may be applied by extension to other criminal organizations with a similar structure, such as:
- Mafia-like criminal organizations based in Southern Italy, such as the Camorra in Campania, 'Ndrangheta in Calabria, Sacra Corona Unita in Apulia, or Stidda or Cosa Nostra in Sicily. .
- Mafia-like criminal organizations based in other countries, such as the Albanian Mafia, Irish Mob in the USA, Organizatsiya in Russia, Yakuza in Japan, Triad society in China, Pashtun Mafia in Pakistan, Dixie Mafia in the southern USA.
