Helen McCarter is living the dream, with a beautiful house and a marriage to Charles, a rich and successful man. However She does not have a job and It is revealed that Charles has had multiple affairs and two sons , An emotionally destroyed and penniless Helen turns to her sassy and loud grandmother Madea, who takes her in and helps her get back on her feet.
Helen soon learns to grow through her pain, and is ready to move on from Charles and give love another chance when she meets Orlando. But when a dramatic turn of events leaves her estranged husband alone and facing death, Helen wonders if she truly has it in her heart to forgive him. In the end, Helen forgives Charles but ends up finalizing her divorce from him, realizing she is in love with Orlando. Helen finds Orlando and confesses her love for him and Orlando proposes to her.

- Mablean Ephriam, former presiding judge of TV's Divorce Court, makes a brief cameo appearance in the film as the judge who places Madea on house arrest.
- The sequel, Madea's Family Reunion was released on February 24, 2006, almost exactly one year after the first film. And Tyler Perry's new movie, Daddy's Little Girls, came out on February 14, 2007, almost one year after the previous one.
- Both Kimberly Elise and Shemar Moore each now star in separate CBS dramas. (Elise in Close to Home (TV series), and Moore in Criminal Minds).
- Madea goes into Brenda's closet flips out when she sees a wire hanger. This is a parody from the movie Mommie Dearest (film)
- The title of the movie pays homage to the title of Ozzy Osbourne's album "Diary of a Madman (album)"
- Tyler Perry stated on "The Oprah Winfrey Show" that his real house was used in the film to save money. However, once people knew what his house looked like and where it was, he had to move because fans were jumping over his fence and tour buses would stop outside.

On its opening weekend, the film arrived at #1 with $21,905,089. The movie grossed a total of $50.3 million in the United States and Canada while it grossed $19,000 in foreign countries adding to a worldwide total of $50,652,203. It was later broken by his second film, Madea's Family Reunion with $63 million.
