U.S. Route 221 runs northeast-southwest (signed north-south) in the U.S. state of Virginia. It enters Grayson County from North Carolina with U.S. Route 21 south of Independence, turning east there with U.S. Route 58. US 221 and US 58 pass through Galax and then enters Carroll County. At Hillsville, US 221 leaves US 58 to head northeast and intersects the southern terminus of State Route 100 just east of Hillsville. US 221 generally parallels the Blue Ridge Parkway through Carroll County, Floyd County, and southwest Roanoke County. US 221 intersects State Route 8 in Floyd. US 221 enters Roanoke as Brambleton Avenue SW and runs on several other roads before leaving downtown Roanoke on Williamson Road. US 221 overlaps U.S. Route 460 from Orange Avenue NE in Roanoke east to Bedford, and then uses an old alignment of US 460 to its end in Lynchburg at U.S. Route 29 Business (the Lynchburg Expressway) while concurrent with U.S. Route 460 Business and U.S. Route 501 Business.
